F-1/J-1 check-in for change of status applicants

If you applied for a change of status from USCIS to either F-1 or J-1 student, please complete the form below and upload copies of your USCIS receipt notice or approval notice.  

  • If you don't have access to a scanner, but you have a smartphone, you can download a free app such as CamScanner for Android or iPhone. You can also take photos with your phone or computer's internal camera. 
  • If you don't have a way to create scans of your documents, you can use library equipment or the computer/scanner at OIA during office hours.  

Once your application is approved, please submit the following documents to OIA:

  • Change of Status receipt or approval notice (see below for a sample)
Personal Information
Please enter your name as it is listed on your I-20 or DS-2019.
Please enter a 10-digit U.S. phone number where you can be reached. Do not include hyphens, dots, or dashes. Note that failure to provide a valid phone number will prevent OIA from registering your record in SEVIS, which will have a negative impact on your immigration record and cause you to be out of status.

U.S. Address Update Confirmation

In order for OIA to register you in SEVIS, you must update your US address on my.UChicago AND set it as the 'SEVIS - U.S' Address by completing the steps below. If you are a student in the Booth School of Business you must update your address on the Booth Application Status Page under “verify address”.


  1. Log on to my.UChicago.edu (be sure to create your CNet ID first).
    Booth students: Update your address on the Booth Application Status Page under “verify address”.
     
  2. Follow the SEVIS U.S. Address Update instructions (Click on the link. Instructions will pop up in a new window). Enter your U.S. address according to the USPS address formatting instructions, which are very specific.

    BOOTH STUDENTS: Update your address on the Booth Application Status Page under “verify address”.

Failure to follow the exact formatting instructions will prevent us from registering your record and will result in the termination of your F-1/J-1 record.
