F-1 and J-1 Student Transfer Out Form

If you are transferring to another U.S. institution, you will need to complete this form. Upon receipt of the form, your adviser will release your SEVIS record to your new school based on the release date you indicate below. If you have a form that needs to be filled out by our office for your new school, you can upload it below.

After completing this form, you should speak with your new school about additional steps you need to take in order to complete the transfer process. Make sure you review all relevant SEVIS Transfer information before completing this form.

Personal Information
Enter your family/last name exactly as it is written on your I-20/DS-2019.
Enter your given/first name exactly as it is written on your I-20.
Enter your 8-digit UChicago ID Number (Starts with '10' or '12')
You will receive a confirmation of this submission at the address provided here.
New School Information
Enter the complete name of your new school, as it appears in SEVIS. You may need to request this information from your new school's international office.
Enter the U.S. city and state where your new school is located.
Enter the SEVIS school code (with 3-digit suffix) of your new school. Sometimes it can be found on your school's transfer in form, but you may need to request this information from your new school's international office. Some schools with multiple campuses may have several different SEVIS codes, so make sure you are using the correct one.
To be eligible for a transfer, you must have been accepted into the new school. Do not request the transfer if you have not yet been accepted. In some cases, OIA may request that you provide official documentation showing that you have been accepted, such as an acceptance letter.
Enter the date when your courses will begin at your new school. This is the date that will go on your new school's I-20, so you may need to confirm this information with your new school's international office.
SEVIS Transfer Request
If you are graduating, the release date you choose below should not be before the end of your last term.
This is the date your new school will have access to your record. See the guidance for selecting a transfer release date, on our website Students on OPT/AT: The release of your SEVIS record will cancel the remainder of your OPT or AT authorization as of the date requested above. Please contact your adviser if you have questions. F-1 Students: You must begin classes in another full-time program within 5 months of your I-20 program end date or the end of your OPT to remain in status. J-1 Students: You must formally check-in with your new sponsor within 30 days of your transfer-out date.
Most school's required you to provide a transfer-in form that is completed by OIA. If your new school requires a University of Chicago DSO/ARO to complete a form verifying your status, you can upload a copy here.
