Request for F-1 CPT Work Authorization

Use this form to apply for Curricular Practical Training (CPT).  Make sure to review all information about CPT work auhtorization before submitting your application. 

CPT will take about one week to be approved (sometimes longer during peak seasons).  You will receive an email once your new I-20 is ready to pick up or if additional information is needed.  You cannot begin work until your CPT has been approved, you have the I-20 showing your work authorization, and the CPT start date printed on your I-20 has been reached.

Please note that your requested CPT start date must be after today.
This is not your offer letter*. A signed offer letter from your prospective employer, on letterhead stationery, giving your job title and duties, the precise beginning and end dates of employment, and specifying whether the employment will be part-time (20 hours per week or less) or full-time (more than 20 hours per week). It also must state the EXACT location of the training, which means the entire address including: street, city, state, and zip code. *Important note: Your employer letter must contain all information listed on the Employer Letter Template 99% of the time your offer letter will not suffice for CPT application purposes. If you are going to submit an offer letter, please highlight where all of the required information is on the letter. Note that submission of an insufficient employer letter will delay your application processing.
For a degree requirement (SSA students): Provide a letter from your Dean of Students referring to the employer's letter and to the degree requirement, and confirming that your job will satisfy that requirement, in full or in part.
