Returning from a Leave of Absence (More than 5 Months)

If there will be more than a five month gap between your enrollment, you will require an 'Initial' I-20.  This will be a completely new F-1 record.

Contact OIA ~3 months before you plan to return to the University.  This will allow time for the creation and receipt of your new SEVIS record/I-20.

You must also coordinate with your department to submit the completed Academic and Financial Review (AFR) form and supporting financial documentation to OIA.  You will be required to provide financial documentation to cover the amount listed as the “Difference*” in the lower right corner of page 2 of the AFR. We do not require physical copies of the financial statements.

Required Documents

For any funds that are not in your name (including parents or spouses). This letter must show the date written, the name and address of the sponsor(s), and the relationship of the sponsor(s) to you (e.g. 'mother', 'friend'). The letter also needs include the sponsor's signature (not just a typed name).
