H-1B TN E-3 Sponsorship Request Forms

Department administrators should download and complete the Sponsorship Request Form and instruct beneficiaries (employees) to complete the Beneficiary Information Form. If sponsoring in the H-1B or E-3 status the Deemed Export Control Attestation shoiuld be completed by the department as well.

Once all forms are complete, department administrators should save their completed forms to Box and provide a shared link via email to international-affairs@uchicago.edu.
