J-1 Scholar Transfer Out Request

To request a transfer of your SEVIS record, please download and complete this form, read the instructions, and email the form, attaching the required support documentation to international-affairs@uchicago.edu. 

Important Note

This form must be completed using Adobe. You will experience issues completing the form if you use any other software or online form filler. An incomplete form is an incomplete request which will delay our ability to process the transfer.

J-1 Transfer eligibility:

  • Must be Active in J-1 program

  • Must have time remaining in J-1 status:

    • Research Scholar/Professor: 5 years maximum participation

    • Short-Term Scholar: 6 months maximum participation

  • Specialist: 1 year maximum participation

  • Research or teaching field at new institution must correspond to current "Subject/Field Code Remarks".

  • Start date at the new institution must be the day immediately following the last day at UChicago. 

  • Must have legitimate offer from Transfer Institution.

  • 212e waiver not initiated

  • While the transfer is pending, international travel is strongly discouraged. Please consult your adviser if you have international travel plans.
